Support services
The team can offer packages of outreach support to individuals within their own homes and within the local community enabling them to live as fulfilled and independent a life as possible. The support sessions will be flexible and responsive to individual’s needs.

Support includes:
- Encouragement and support to develop and expand social networks (e.g. re-establish contacts with family and friends)
- Encouragement and support to access community resources and reduce social exclusion and isolation (e.g. explore local facilities, choices and available options)
- Accompany service users to appointments, shops, banks etc. with future goals of attending alone (e.g. tailored interventions and reducing support over a planned time frame)
- Practical coaching in daily living skills to increase confidence to live independently (for example cooking, cleaning and budgeting)
- Encouragement and support to access volunteering opportunities
Individuals would work with an identified key worker and their progress would be monitored through an Individual Support Plan tailored to the individual’s agreed areas of support. This work will link closely with the individual’s CPA and will be reviewed on a regular basis. We will provide an audit trail of support and engagement to the individual Care Co-ordinator. The service is run on an Individual service contract basis or personal budget.
