We offer packages of outreach support to customers in their own homes, helping them to live as fulfilled and independent a life as possible, with flexible support sessions responsive to individual needs

Mental health outreach services

We offer support to:

  • develop and expand social networks (e.g. re-establish contacts with family and friends)
  • access community resources and reduce social exclusion and isolation (e.g. explore local facilities, choices and available options)
  • attend appointments, shopping, banks etc. with future goals of attending alone (e.g. tailored interventions and reducing support over a planned time frame)
  • increase confidence to live independently (for example coaching for cooking, cleaning and budgeting)
  • access volunteering opportunities

A Key Worker will monitor the customer’s progress using an Individual Support Plan tailored to agreed areas of support. This work is reviewed on a regular basis, including an audit trail of support and engagement to the individual Care Co-ordinator.
