Supporting customers to move on to independent accommodation
Broadacres is successful when supporting customers to move on to independent accommodation within the timeframes set out in the service specification.
We continually monitor progress to make sure moving to more independent accommodation is sustainable, and we support customers to complete a:
If appropriate, we offer up to six months ‘Floating Support’ that includes establishing links with local housing teams to find a home, support for accessing housing options and six weeks of follow up support and progress will be monitored at the monthly multi-agency panel meetings.
